Figure 2) Make a cut as close to scalp as possible. Obtain proper amount of hair from the crown of the head. Maintain firm hold of the cut sample.
Figure 3) While maintaining hold on hair sample, use the ruler provided on the hair specimen pouch to measure and cut the sample at the 1.5" marks from the root end. Discard all hair beyond 1.5".
Figure 4) Place only 1.5" specimen of hair in provided pouch.
Figure 5) Seal hair pouch with small security label provided on the Chain of Custody form. Place sealed collection pouch into the inner pocket of the clear specimen bag. Place the first three (3) copies of the Chain of Custody form into the outer pocket of the clear specimen bag. Remove the blue backing from the clear specimen bag and seal the bag.
*** If necessary, obtain additional samples of hair from other regions of the head.
*** With shaved/short hair, locate longest hair possible and make multiple cuts from different locations to produce sufficient amount of hair needed.
*** Do not mix head hair and body hair together. If sufficient head hair is not available, collect other body hair. Please specify on C.C.F. if sample is body hair.
Lab needs approximately 90-120 strands of hair.
